PAYMENT AND INSURANCE INFORMATION

Prior to your first consultation, Dr. Herman’s administrative team will coordinate with you to determine your medical insurance coverage for the treatment you are seeking. Patients should be aware there may be out-of-pocket expenses during the course of treatment.

If you will be utilizing medical insurance, review your medical insurance policy coverage by calling the customer service number on the back of your insurance card. For patients paying out-of-pocket, we accept personal checks and major credit cards, with the exception of American Express, but cannot accept cash.

MEDICAL INSURANCE DEDUCTIBLE

Since the Affordable Care Act (Obamacare) was instituted, most of our patients are now required to pay a deductible before medical insurance coverage kicks in. (Typically this amount is several thousand dollars, depending on your plan’s coverage. Check with your insurance company for your personal and family deductibles).

Like many medical practices today, when we accept a new patient, we ascertain the status of their yearly deductible. If it has not been fulfilled, we ask that patients pay our fees at the time of service, per our practice policy.

Keep in mind, after you submit your (paid) medical bill to your insurance company, it will be credited toward your deductible. If it is determined once you have met your deductible that you have overpaid, a refund will be provided to you immediately.

APPOINTMENT CANCELLATION

We try to be very efficient and accommodate patient appointment requests as quickly as possible. However, failure to give 24-hour notice of cancellation of an appointment or not keeping your appointment will result in a charge of $75.00. This charge is not covered by your medical insurance company and is your fiscal responsibility.
